  • The Procedure

    - Multiple Sclerosis, muscle spasms or spasticity.

    - PTSD.
    If you do not have one of these qualifying conditions, you must first book a wellness assessment to be evaluated by our team of medical professionals to determine the next steps.

  • The Results

    From gathering medical records to submitting your state application, we’ll walk you through each step until you receive your Kentucky Medical Cannabis Card. After your exam, you will receive confirmation regarding your eligibility for a KY Medical Cannabis Card. Approval of qualifying medical conditions for Kentucky’s Medical Cannabis Card Program is determined solely by our nurse practitioner following a thorough evaluation.     2. The medical cannabis practitioner is not authorized to issue a certification;
    3. The medical cannabis practitioner has reason to believe that the patient or a caregiver is abusing or diverting medicinal cannabis; or
    4. The patient is deceased.
    If the practitioner is unable to conform to the professional standards (Standards of Care) established due to circumstances beyond their control, or if the practitioner makes a professional determination that it is not appropriate to comply with a specific standard based upon the individual facts applicable to a specific patient's diagnosis and treatment, the practitioner shall document those circumstances in the patient's record and only provide a written certification to the patient if the patient's record appropriately justifies the use of medicinal cannabis under the circumstances.
